Applicant Acknowledgement: By submitting an application* for the Fall 2023 CAQHI Program, the applicant acknowledges that they are applying simultaneously for BOTH of the below courses (no need to apply separately for each course), which are requirements of the certificate program and associated specifically with this respective CAQHI Program offering (Group: CAQHI-3):
*An accepted CAQHI Program application provides the applicant with automatic enrollment in both courses, provided that all stated course requirements are successfully completed. HOWEVER, prior to the start of each course, learners enrolled in the CAQHI Program are required to confirm their continued intention for enrollment/participation in the respective course. To do so, their prompt attention and reply to I-QIPS communications sent to them in this regard is kindly requested.
Note: The above courses are also available separately and without enrollment in the CAQHI Program. An applicant who is interested in taking only one of the courses, or would like to take both courses without participating in the CAQHI Program, should apply separately for each respective course of interest. To do so, visit the Education page, where Calls for Applications (by course) are announced when available. The QI Jump Start course is the first of two required courses for the CAQHI Program. This intermediate-level course builds upon foundational skills, as well as those concepts acquired from each course session.

The Leading Healthcare Improvement (LHI) course is the second of two required courses for the CAQHI Program. This advanced-level course is designed to develop leaders of interprofessional improvement teams within their clinical settings.

As the final required step of the CAQHI Program, each enrolled learner is responsible for completing and submitting one (1) healthcare improvement capstone project.
The successful completion of the capstone project is based on the continuity of the project (in its development and implementation) across both of the CAQHI Program-required courses, as well as per stated CAQHI Program criteria for completion, which includes the items specified in the Capstone Project Requirements and Criteria section below.
The completed capstone project should be leadership-approved and a culmination of the learner's applied learning over the term of the certificate program (across both required training courses).
Further, the project should include at least one (1) full/complete Plan-Do-Study-Act (PDSA) cycle (not including baseline data collection), and data from the PDSA cycle must be presented in the required abstract submission described below.
Each of the below items is required for the capstone submission process. You will be prompted to enter or upload these items when completing the Capstone Submission Form noted above (Step 2).

The submitted healthcare improvement Capstone Project (Abstract), described above, will be evaluated based upon the Capstone Evaluation Rubric provided with the Capstone Submission Form noted above (Step 2).
