Certificate of Added Qualification in Healthcare Improvement (CAQHI) Program

For questions, please contact: I-QIPS Team

Program Enrollment

Enrollment in the Certificate of Added Qualification in Healthcare Improvement (CAQHI) Program offered by the Institute for Continuing Professional Development in Quality Improvement and Patient Safety (I-QIPS) at Baylor College of Medicine (BCM) is contingent upon official written acceptance to the program and, if applicable, payment of the program registration fee in full no later than the due date specified on the invoice. Partial payments are not accepted. Any partial balances received will be voided or refunded, and registrations with outstanding balances past the invoice due date are subject to immediate cancellation.

Cancellations & Refunds

Requests to cancel enrollment or receive a registration refund must be submitted in writing and received by the I-QIPS no later than 10 business days (by 11:59 PM CT) before the start date (Session 1) of Course 1 (QI Jump Start) of the CAQHI Program. Business days are defined as Monday through Friday, excluding official holidays observed by BCM. The date the request is received by the I-QIPS will be considered the cancellation date.

Requests received after the refund deadline will not be processed, even if no course sessions were attended or materials accessed. To cover guarantees and other programmatic expenses, approved cancellations or refunds are subject to a non-refundable administrative fee (10% of the program registration fee), which will be deducted from the original payment amount received.

Requests must be emailed to the I-QIPS (CCIT-mgmt@bcm.edu) and include the invoice number, program name, and the participant’s full name and email address (both as enrolled in the CAQHI Program).

The I-QIPS reserves the right to modify the CAQHI Program at any time, including cancellation of a program offering due to extenuating circumstances. Program participants will be notified in writing at the email address indicated on their application form or invoice. If a program offering is canceled by the I-QIPS, liability is limited strictly to a refund of any original payment amount received.
