When you attend and/or participate in a Baylor College of Medicine Institute for Continuing Professional Development in Quality Improvement and Patient Safety (I-QIPS) activity (including, but not limited to, meetings, training sessions, programs, conferences, other events), whether in person or remotely, you enter an area where photography, audio recording, and/or video recording may occur.
By attending/being present at/participating in an I-QIPS activity, you are acknowledging ALL of the following:
- You understand that photography, audio recording, and/or video recording may occur during the activity.
- You understand that your presence at the activity, regardless of your role, constitutes your consent to be photographed, filmed, and/or otherwise recorded and to the release, publication, exhibition, or reproduction of any and all recorded media, voice, and name for any lawful purpose whatsoever in perpetuity in connection with the I-QIPS and its initiatives, including, by way of example only, use on websites, news/announcements, and in social media, such as for promotional, reporting, and/or educational purposes.
- You release Baylor College of Medicine, the I-QIPS, its affiliated faculty, staff, and each and all persons involved from any liability connected with the taking, recording, digitizing, and/or publication and use of photographs, computer images, video recordings, audio recordings, and/or any other such media.
Further, you waive and release all rights you may have to any claims related to the use of any recorded media of you during the activity, including, without limitation, any right to inspect or approve the photograph, video recording, or audio recording of you; any claims for invasion of privacy, violation of the right of publicity, defamation, or copyright infringement; and/or any fees, payments, or royalties in connection with the use of such record media.
- You understand that all photography, filming, and/or recording will be done in reliance on this consent.
- You agree that you have been fully informed of your consent, waiver of liability, and release before attending/being present at/participating in the I-QIPS activity.
