QI Effort Types & Deadlines
The BCM MOC Program offers several pathways (or QI Effort Types, as described below) for physicians and physician assistants (PA) to earn MOC Part 4 credit/points or NCCPA Category 1 PI-CME credit, respectively, for qualified QI activities.
When submitting requests for credit consideration, time is of the essence. As such, physicians/PAs are responsible for ensuring they meet BCM MOC Program participation submission requirements and associated deadlines for the respective QI Effort Type through which they are seeking credit.
PARTICIPATION SUBMISSION DEADLINES
IMPORTANT DATES
Unless otherwise indicated, the BCM MOC Program accepts participation submissions on a monthly basis from physicians/PAs to request credit consideration for their engagement in qualified QI activities. However, any physician/PA seeking credit for the CURRENT calendar year, must submit their participation submission by its respective year-end deadline. Monthly and year-end deadlines are indicated below.
The submission process typically requires the physician/PA to attest to and reflect on their participation in the QI activity (via a Physician/PA Attestation & Reflection Form); however, other information also may be required for certain QI Effort Types. Each QI Effort Type has its own respective Attestation & Reflection Form, which must be completed/signed by the physician/PA seeking credit and submitted by its associated deadline (which may vary across QI Effort Types), in order to be processed for credit consideration.*
The BCM MOC Program uses the form data to prepare completion notifications and other documentation required for submission to the ABMS Portfolio Program for credit consideration. If approved, credit is recorded by the relevant participating ABMS Member Board(s) or the NCCPA for the year in which they receive the physician/PA completion notification.

YEAR-END DEADLINE
October 15, 2026
To request credit for the 2026 calendar year, the following forms (as relevant to the QI Effort Type of interest) must be submitted by the specified deadline, unless otherwise noted:

*Before a QI Activity - Physician/PA Attestation & Reflection Form can be submitted, the corresponding New QI Activity Application & Project Lead Commitment Form for the activity must first be pre-approved and on file with the BCM MOC Program. See section below for more information about earning credit for a QI Activity.
If the Year-End Deadline is missed, contact the BCM MOC Program to determine what steps are available (if any) to obtain credit in the subsequent calendar year.
